Sports physical questions.
Do I need an appointment for a sports physical?

Bring the form your school or league gave you. Louisiana high school athletes use the LHSAA preparticipation physical form.
Does a parent need to come?
If the athlete is under 18, a parent or guardian should come to the visit and sign the form.
Do you do physicals for middle school, club and rec league sports?
